Example of Those Who May Qualify For Hospice Care:

• Patients suffering from incurable terminal illnesses

• Patients undergoing cancer treatment with no improvement or     

   deteriorating health, leading to discontinuation of further treatment

• Patients with an expected life expectancy of less than 1 year due   

   to ongoing illness

• Patients with mobility limitations preventing regular visits to doctors

• Individuals suffering from chronic illnesses for an extended period

• Patients no longer desiring repetitive hospitalizations, surgeries, or   

   tests for treatment purposes

• Patients who, after consulting with family, choose pain relief and   

   symptom management

• Patients severely affected by dementia experiencing difficulty in     

   eating or significant weight loss over a period of time

Hospice Services Include:

• Control of pain and other symptoms through medication,

    environmental adjustment and education

• Psychosocial support for both the patient and family,       

    including all phases from diagnosis through bereavement

• Medical services commensurate with the needs of the patient

• Interdisciplinary "team" approach to patient care, patient   

    and family support, and education

• Integration into existing facilities where possible

• Specially trained personnel with expertise in care of the     

   dying and their families
